These questionnaires are one of my favorite things to do with my kids. And they’re a must to save in memory boxes for your kids to read when they’re older. Some of the answers to the questions can be out of this world funny. Ahh to see the world in the eyes of a little kiddo!

This questionnaire consists of 20 questions for your child to answer about Dad. Every question from what is your Dad’s favorite drink to what do you think Dad was like as a little boy? Expect hilarious but heartwarming answers!
